What Stress Actually Does to the Body

Understanding why Physio-Led massage works so well for stress management requires a brief look at what stress does to the body in the first place.

When we are under pressure and perceive a threat — whether that’s a difficult conversation, a looming deadline, a financial worry, or simply a relentless pace of life — the sympathetic nervous system activates.

Adrenaline and cortisol flood the system and we have a systematic response. The heart rate rises. Your central nervous system activates and muscles contract and hold, particularly in the neck, shoulders, jaw, and lower back ribcage. Breathing pattern changes and becomes shallower higher in your chest.

In the short term, this response is adaptive — it’s the body preparing to take action. But when pressure of everyday life and worries mounts and mounts, over time, the stress effect becomes chronic, as it is for a growing proportion of adults in the UK, the entire system never fully switches off. Continuously running on idle, the brain and nervous system keep firing the same patterns, of over thinking, worrying, muscles remain tight, breathing shallow and  Cortisol stays elevated.

This cascade impacts every system, sleep quality deteriorates, the immune system is suppressed and what began as a temporary physiological response becomes a sustained state that damages health from multiple directions simultaneously.

This is the context in which Clinical Physio-Led Massage and Bodywork operates — and why its effects go considerably further than simple relaxation.

The Science: What the Research Actually Shows

In 2020, researchers at the University of Konstanz in Germany published findings that provided some of the clearest scientific evidence yet to support what many Physiotherapists who practice clinical massage have long understood in clinical practice.

The study, led by a neuropsychology doctoral student was overseen by a professor of the university’s neuropsychology laboratory.

They directly set out to review whether they could induce physical and psychological relaxation through massage.

They compared individuals receiving massage with individuals sitting and consciously relaxing and measured the impact on the parasympathetic nervous system — the body’s built-in recovery and restoration mechanism, and the precise counterpart to the stress response.

Participants’ responses were assessed using a combined qualitative and quantitative methodology: physiological measurements — heart rate and heart rate variability — and psychological self-reporting.

The results confirmed that massage measurably activated the parasympathetic nervous system to a significantly greater degree than sitting resting alone.

Crucially, the effect was not merely participants self reports, subjective. It was captured in objective physiological data.

Heart rate variability is a reliable marker of parasympathetic activity and therefore a measure of genuine relaxation and stress recovery, and this improved meaningfully more in the massage groups.

To understand and address the damaging effects of high pressure and stress we need to promote the opposite: psychophysiological relaxation.

This study demonstrated that massage is one of the most effective ways to produce that relaxation state, and that it does so through measurable, reproducible biological mechanisms rather than through placebo or perception alone.

This matters enormously for individuals in Reading under pressure seeking stress management support.

It means that Physio-Led Clinical Massage is not simply making someone feel calmer in the moment — it is a clinical tool, an evidence supported way to actively shift the body and mind from a sympathetic-dominant stress state into a parasympathetic-dominant recovery state.

One over-pressured person can habitually ‘carry stress’ around the head and neck, showing up as Tension Headaches, Migraines, TMJ problems, eye strain, neck and shoulder pain and restricted cervical movement.

Another develops persisting, relapsing and remitting episodes of back pain, gut problems (DGBI, IBS), disrupted sleep (insomnia) and limitation of physical activity.

A third may have non-cardiac related chest pain, restricted thoracic mobility, rib pains, reduced breathing depth, which in turn sustains the shallow breathing pattern that keeps the sympathetic nervous system activated.

A fourth develops jaw tension, pain in the face, and temporomandibular dysfunction, teeth grinding (bruxism).

In each case, the presenting physical pattern reflects a different stress signature — and effective Physiotherapist-Led clinical treatment requires assessing, identifying and addressing that specific signature rather than applying a generic protocol.

Chronic neck and shoulder tension:

This is among the most universal physical manifestations of stress. The upper trapezius, levator scapulae, and suboccipital muscles all respond to sympathetic activation by contracting — a relic of the protective bracing response.

In chronically stressed individuals, these muscles rarely fully release, producing persistent tightness, restricted neck movement, and repeating tension headaches that can become daily or near-daily.
