London Case Manager was founded in 2023 by Jill Wigmore-Welsh an extended scope of practice Physiotherapist & Psychotherapist with 45 years specialism in neuro-orthopaedic rehabilitation, and 15 years experience providing legal services and working as a case manager. London Case Manager are a completely independent, London based firm, who mainly, but not exclusively, specialise in the provision of high quality, personally tailored, case management and vocational rehabilitation services within Greater London TFL zones 1-7 and the Elizabeth Line region for adults who have suffered catastrophic and life-changing injuries. Instructions are accepted as single or joint instruction under the Rehabilitation Code (2015) or direct by the injured party or their representative.

When required, we are on hand supporting the client to fully participate in decision making, and whatever it takes, to be as independent as possible in their everyday activities, including enabling their participation in education, work, recreation and life roles they passionately enjoy.

    We mainly, but not exclusively, specialise in rehabilitation service provision within the geography of Greater  London TFL zones 1-7 and the Elizabeth line region

    We endeavour to use independent providers with good practice facilities and a minimum of 10 years experience in their field .

    We match clients interests and preferences with professional providers to facilitate their full engagement with the rehabilitation process.

    We ensure rehabilitation providers submit initial and follow up evidence supported reports through provision of necessary training.

    We request and review reports for objective evaluation of input and outcome with budget agreed.

    Our referral process:

    • Enquires can be made via emailtelephone or through the website via our contact page.

    • The injured person’s needs will be reviewed, and you will be provided with a CV, together with a quote, and terms of business within 48 hours.
    •  
    • Should further discussion be necessary you will have the opportunity to discuss further with Jill Wigmore-Welsh. We also offer options for arranging a ‘Meet and Greet’ webcall for your client and this can be reviewed on a case by case basis.

    • Once agreed, Jill Wigmore-Welsh will make telephone contact with the injured person within 2 working days to arrange an initial appointment.

    • Before the meeting the injured person or representative will be sent information about London Case Manager and case management to ensure that they fully understand the INA and the case management process. They will also be provided with a consent form to agree to release of appropriate medical information and sharing of information with appropriate parties, as part of the INA and any further authorised case management interventions.

    • Jill Wigmore-Welsh will then either visit the injured person in their home, or provide a webcam consultation, within 10 working days to undertake the assessment.
    •  
    • The report, inclusive of outcome measures, predications, and costs, is prepared and sent out to the referring parties, within 10 working days of the assessment. Should an urgent request be made it would be recommended that this is discussed and agreed as part of the referral enquiry.
